Terms and Conditions for After-School Chinese Club at Victoria Avenue School

Participant: I hereby authorize my child, whose name is provided on this form, in the specified room number, to participate in the after-school Chinese Club at Victoria Avenue School.
School Policies: I have read, understood, and agreed to adhere to all of Victoria Avenue School’s rules, policies, and procedures.
Fees: I acknowledge that there is a term fee associated with this extra-curricular program. Payments should be made to the following bank account: 01-0270-0564711-01. The lesson fees are charged on a per-term basis and refunds are only applicable for classes that are cancelled at the discretion of the teacher or the school.
Conclusion of Lesson: I commit to adhere to the option selected on the form for the collection of my child at the conclusion of the lesson, either by myself or by the staff of Magnificent Kids.
